Project SCOPE has been designated by the New York City Department for the Aging as the case management program for non-Medicaid eligible homebound older adults residing between East 14th and East 96th Streets, including Roosevelt Island. The program is open for information and referrals Mondays-Fridays. Financial Management Services for Older Adults (60+): Monitoring monthly income and expenses, payment of monthly bills and medical insurance reimbursement assistance provided for older adults who have difficulty managing their own finances. The type of management provided is tailored to individual needs. Fees for services are on a sliding scale based on an individual's ability to pay. Please call (212)744-5022 x1203. East Side Transportation Services: A transportation service administered by LHNH. Referrals are accepted through Project SCOPE Intake, 744-5022 x1203. Transportation is available through the LHNH wheelchair accessible bus; car service and taxi fare reimbursements are also available. Services are provided seven days per week. The Caring Neighbor: Provides personal care services to over 450 elderly and disabled, Medicaid-eligible individuals. Home attendants provide assistance ranging from 4 hours per week to round-the-clock care, depending upon individual need and availability of funding. Referrals are made through HRA (CASA Office), Project SCOPE, or the New York Foundation for Senior Citizens (for clients living on the Lower East Side and in Chinatown). Lenox Hill Senior Center (60+): Located at 343 East 70 Street, offers free programs including creative writing, foreign languages, current events, arts and crafts, exercise classes and classical music. The Senior Companion program provides escort services and companionship to frail Senior Center members. A nutritious lunch is available 7 days a week at 12 noon for a suggested nominal contribution. Please call (212)744-5905. Senior Center at St. Peter's Church is located at 619 Lexington Avenue at 54th Street. The center operates Mondays and Wednesday through Saturday. Lunch, information and referral, counseling, escort and transportation are offered. Discussion groups, languages, exercise, the arts, and special activities are all offered to center members. Must be 60 years of age or older and live in one of the five boroughs of New York City to be eligible as a member. Suggested contribution for meals and transportation. Adult Day Services at Lenox Hill Neighborhood House consist of two different programs: the Health Enhancement Partnership (HEP) and the Center for Alzheimer's Respite for the Elderly (CARE Program). Health Enhancement Partnership (HEP) (60+): Social adult day care program at the Senior Center for physically and/or mentally frail adults. Individual-centered programming with full-or half-day participation, 1 to 5 days per week. Members enjoy exercise and movement therapy, music enrichment, arts and crafts, intergenerational exchanges, cultural and recreational trips, and parties in a stimulating, supportive environment. Participants in HEP are no longer socially isolated and often show improvement in nutritional intake, behavior, mood, self-esteem and cognitive functioning. Transportation may be arranged when possible. Participant contributions are requested. Referrals may be made by calling Lindsay Goldman at (212)744-5905 x106. The Center for Alzheimer's Respite Care (CARE) program is designed to provide assistance to individuals who suffer from Alzheimer's and related dementias. The CARE program operates five days a week, from 10:45 am-2:45 pm. The program is held at All Saints Episcopal Church, 230 East 60th Street. Support Training Advocacy and Respite (STAR) provides individual counseling and support group services for individuals experiencing caregiver stress. Caregivers Legal Support Center provides legal information, advice and representation to unpaid family caregivers to manage the compex needs of their frail elderly family members and care recipients. The Legal Support Center provides integrated legal services on a variety of civil legal issues that directly relate to helping improve the quality of life of the care recipient, such as housing, medical benefits and health law, income support and advance planning documents. In addition to direct legal representation for people who are economically disadvantaged, the Legal Support Center educates and counsels caregivers and care recipients on topics such as advance directives, housing, supplemental needs trusts, Medicaid and home health care planning. We also collaborate with other caregiver organizations throughout New York City by sharing information, providing referrals and reaching out to more caregivers and care recipients in need of assistance.
