Services:
Care Management Services (comprehensive clinical services to older adults and their families). Evaluation and assessment of physical, functional and social abilities of older adults, coordination of services and ongoing monitoring of home care and placement alternatives. Counseling and psychotherapy. Financial management services to assist with monthly household bills and medical insurance claims. Education and counseling to alleviate the strain and stress experienced by adult children and family members. Assist families with long distance care giving and support. Information and referral. In-home consultations available for older adults and the disabled.

Fees:
Private pay for all care management services. Most psychotherapy/counseling services are Medicare eligible.

Cancer/Alzheimer's disease; HIV and AIDS experience). Court appointed evaluator and guardian (Article 81 of the Mental Health Law).
