Services:
CAPP Caregiver Resource Center Services: Information and referral, counseling for family caregivers of adults and older persons. This program helps family caregivers of adults and healthcare workers seeking information about caregiver resources. It is staffed with social workers who understand the special commitment the caregiver has made. If you are concerned about handling the responsibilities of being a caregiver or would like to learn more about programs to help caregivers, please call CAPP. Very limited funds are available to help caregivers of qualified elderly pay for respite care and other services. There are no geographic restrictions or fees for services although contributions are welcome. For referrals or more information, please call (212)241-2277. Serving New York City Metropolitan area. Geriatric Psychiatry: This is a multi-disciplinary group providing comprehensive mental health treatment of older adults. The clinic offers evaluation and management of care to persons 60+ years of age, and assistance to their caregivers. Expert physicians, nurse practitioners and social workers are available for the evaluation of each patient. The Psychiatry Clinic specializes in: memory disorders, behavioral disturbances that result from memory disorders, depression, chronic mental illness, individual and family counseling. The Caregivers Program, available in English and Spanish, reaches out to caregivers of dementia patients via in-person sessions and telephone conference call workshops. The Alzheimer's Disease Assistance Center (ADAC) enhances dementia patients' access to medical and psychosocial services. ADAC is available to provide talks at senior centers and other community organizations about memory and aging. ADAC conducts a support group for caregivers of dementia patients in Spanish and a Spanish-speaking telephone education group. Contact Dante Tipiani for more information at (212)241-6197.

Coffey Geriatrics Associates: The comprehensive care offered to mature adults is provided by a "care team" which includes a geriatric medical specialist, a nurse practitioner, registered nurse and certified social worker. The practice offers a full range of preventative treatment, including comprehensive cancer screening and influenza and pneumonia vaccinations, primary care and on-site consultations with other medical specialists. Dementia screening and treatment is also available. If patients require hospitalization, members of the care team follow his or her care, and consult with specialty physicians. In conjunction with the assigned physicians, social workers coordinate care and resources for homebound older patients with the goal of minimizing emergency room and inpatient visits. The Acute Care for the Elderly Unit addresses the special needs of elderly hospitalized patients. Available in all areas. For referrals or more information, please call (212)659-8552. The Elder Abuse program at Mount Sinai Hospital offers support and assistance to elderly clients who are victims of physical, psychological, financial abuse and/or neglect. A social worker helps victims and their families by: exploring the abusive situation; providing information and referrals; developing strategies to prevent further mistreatment; helping family or other caregivers obtain counseling to better manage stress; providing referrals to support groups; following up with reassurance calls; and assisting with legal matters. There is no charge for services.

The Visiting Doctors Program: The Visiting Doctors Program provides medical care to men and women with complex and serious illnesses who have difficulty leaving their homes. The program provides primary care services to a broad range of homebound patients including frail older people; people with dementia, psychiatric illness, neuromuscular disease, cancer, or diabetes; and people who are terminally ill. Primary medical providers are either physicians or Nurse Practitioners. Licensed social workers are available to coordinate care, to optimize resources and to provide supportive counseling. Open to all family caregivers of adults age 18 and over. Covers Manhattan only. Medicare, Medicaid, private insurance, many managed care plans accepted. Services available in English and Spanish.
