The organization has an extensive portfolio of services to meet the diverse needs of their many patients. The services range from the skilled care of nurses, rehabilitation therapists, social workers, mental health clinicians and home health aides. The caregiving has the fundamental goal of helping patients reach the best possible clinical outcome while remaining independent in their own homes and communities. Visiting Nurse Service of New York offers services for older adults through short and long term programs. The short term programs are Acute Care Services and Rehabilitation Services. The wide range of long term services include VNS CHOICE, VNS CHOICE SELECT (a Medicare advantage program), Long Term Home Health Care Program, Congregate Care and Hospice Care. At Home...Where I Belong and Elder Care are two private pay programs that target the needs of older adults.
