PACE (Program of All-Inclusive Care for the Elderly) is a fully capitated Medicaid funded program that provides a full array of medical, day health center and home care related services for those who wish to have a complete multi-disciplinary team provide and coordinate their care. PACE includes a no-cost prescription drug benefit. SELECT is a partially capitated Medicaid managed long term care program (MLTC) that provides nursing case management and other health related services. Eligibility, including Geographic Areas Served: • SELECT program age requirement is 18 years and over • PACE program age requirement is 55 years and over • Medicaid eligible or able to pay privately • Nursing home eligible; yet be able to live safely at home at the time of enrollment • Require at least 120 days of long term care services • Residents of Manhattan, Bronx, Brooklyn, Queens, and Westchester Fees: PACE is a fully capitated Medicaid funded program and SELECT is a partially capitated Medicaid funded program.
