The former provides care management along with a broad range of health, social, emotional, nutritional, and rehabilitation services through a team approach. Eligibility, including Geographic Areas Served: Must be 55 years of age or older. Be Medicaid eligible or private pay. Have health problems and limitations that would qualify client for nursing home level of care. Require long term care offered by Senior Health Partners for at least 120 days from date of enrollment. Be able to live safely at home with the support of Senior Health Partners. Service area: Manhattan or Bronx residents.
