Partners in Care has over 6,000 home healther aides (HHA), RNs and LPNs on staff and is the largest private home care agency in New York. All of our home health aides receive 30 hours of training over the state requirements. We are a licensed and insured agency, with over 20 years of experience. We provide comprehensive in-home and community based services. Our home health aides are trained to provide personal assistance, housekeeping, help with bathing, companionship and medication reminders and cases are supervised by registered nurses.
HHA Services: From personal assistance and bathing to companionship. Skilled Nursing: Private duty LPNs and RNs, hourly or per visit basis. Ambulatory Escort: Home health aide will accompany to medical/professional appointments or community events. "At Home Where I Belong": In-home assessment of clinical and social issues with recommendations for resources and home care services, as well as hourly case management provided by certified Geriatric Case manager. PRI assessments for nursing home eligibility. Corporate and community-based immunization programs, including flu shots, blood pressure and cholesterol screenings.

Five boroughs of New York City, Nassau and Westchester
Fees:
Private pay and long-term care insurance; reduced rate and continuity of care on tandem cases through VNSNY where care is paid for by Medicare and private pay.
