Services:

Geriatric care management and client advocacy since 1981.
We provide comprehensive assessments,care planning and services, helping client and family deal with complex rules, systems, bureaucracies and serious medical decisions. We arrange coordinate and monitor home care and many support services. We also provide:
Advocacy for:
Helping client receive Maximum Medicaid home care hours. We are with the client for all interviews
Triggering long term care insurance benefits.
Emergency rooms, hospital and doctor visits

Counseling and guidance:
Advice and guidance on dementia
Ongoing support to families
Guidance on care options as client's needs change,
Helping clients and families with complex decisions
Counseling and support to client
Monitoring of home care staff.

Paperwork:
Organize and submit documents for catastrophic insurance
Help client gather documents for Medicaid application
Declutter

Entitlements:
Evaluate client for eligibility and assistance in procurement of entitlements.

Eligibilty:

Manhattan, Queens, Bronx, Brooklyn

Fees:

Individual fees for projects.
Hourly rates for ongoing care management.
