East Side Case Management Consortium (ESCMC): Lenox Hill Neighborhood House is the lead agency in the East Side Case Management Consortium (ESCMC), which provides comprehensive DFTA-funded case management services to homebound seniors living on the Upper East Side and East Harlem. ESCMC staff makes home visits and assist clients in accessing services such as home delivered meals, in-home housekeeping/personal care services, financial management, supportive counseling and entitlements and benefits. Catchment area: East 59th and East 143rd Streets, from 5th Avenue to the East River, including Roosevelt Island. To make a referral, contact the intake line at 212.744.5022, ext. 1203, Monday-Friday.East Side Transportation Services: A transportation service administered by Lenox Hill Neighborhood House. Transportation is available through the Lenox Hill Neighborhood House wheelchair accessible bus; car service and taxi fare reimbursements are also available. Services are provided seven days per week.
Weekend Connections: Lenox Hill Neighborhood House runs a weekend activity and transportation program for senior citizens on Manhattanâ s East Side. Transportation is provided for a wide variety of excursions to senior centers, museums, performances, churches, synagogues and other destinations. Center @ Lenox Hill Neighborhood House: Located at 343 East 70th Street, the innovative senior center offers free programs for Center Members including creative writing, foreign languages, current events, arts and crafts, health and wellness, computers and technology, exercise and classical music. A nutritious breakfast, lunch and dinner are available 7 days a week for a suggested donation of $1.50. Please call 212.744.5905 for more information.
Senior Center at St. Peterâ s Church: Located at 619 Lexington Avenue at 54th Street, the center operates Mondays and Wednesday through Saturday. Lunch, information and referral, counseling, escort and transportation are offered. Discussion groups, languages, exercise, the arts, and special activities are all offered to center members. Must be 60 years of age or older and live in one of the five boroughs of New York City to be eligible as a member. Suggested contribution for meals and transportation. The Center for Alzheimerâ s Respite Care (CARE) program is a private-pay social adult day center designed to provide assistance to individuals who suffer from Alzheimerâ s and related dementias. The CARE program operates five days a week, from 10:00 am-2:00 pm. The program is held at Lenox Hill Neighborhood Houseâ s 70th Street Senior Center at 343 East 70th Street. Support Training Advocacy and Respite (STAR) provides individual counseling and support group services for individuals experiencing caregiver stress. Caregivers Legal Support Center provides legal information, advice and representation to unpaid family caregivers to manage the complex needs of their frail elderly family members and care recipients. The Legal Support Center provides integrated legal services on a variety of civil legal issues that directly relate to helping improve the quality of life of the care recipient, such as housing, medical benefits and health law, income support and advance planning documents. In addition to direct legal representation for people who are economically disadvantaged, the Legal Support Center educates and counsels caregivers and care recipients on topics such as advance directives, housing, supplemental needs trusts, Medicaid and home health care planning. We also collaborate with other caregiver organizations throughout New York City by sharing information, providing referrals and reaching out to more caregivers and care recipients in need of assistance.
