Eligibilty:

Individuals with serious chronic health problems as well as individuals recovering from an illness, injury or a recent hospitalization. Eligible clients must be residents of Manhattan.

Fees:

LTHHCP services are provided through Medicaid and are capped at 75% of the annual cost of nursing home placement.

Additional Information:

In addition, the LTHHCP is Medicare certified so in the event of an acute episode, the LTHHCP can continue to take care of the client.
