Services:

SCORE Rehabilitation Physical and Occupational Therapy Services, PLLC are the elite group of “house-call” therapists who provide top quality physical and occupational therapy services to the senior community. Our mission is to provide our therapists with a professional, rewarding, and stimulating career while giving them the tools needed to provide an elite level of care to restore our patients back into the community.

We are NOT a Medicare part A certified home health agency (CHHA) like a visiting nurse service. Our company is based on a Medicare part B model which means our patients do not have to be classified as "homebound." Under this model we can provide extended time limits on therapy to seniors based on their medical necessity once issued a referral from any of their primary medical physicians.

Eligibilty:

Manhattan, Brooklyn, Bronx, Staten Island and Queens

Fees:

Medicare accepted, self-pay options also available
