Services:

Our agency specializes in working with very difficult clients and complex situations. We have contacts across a vast network of hospitals and community resources. Our services include but are not limited to:

Comprehensive Geriatric Care Management and Case Management
Home Visits
Home Care: arranging for aides through agencies; monitoring home care
Entitlement Expertise: Medicaid (including applications), Medicare (including Part D prescription drug coverage), HMOs, private insurances, long-term care insurances
Arranging recreational activities (e.g., museum visits, music groups)
Review of alternate housing options
Health care consulting: referrals to physicians, accompanying clients to medical treatment, referrals to rehab
Managing bills: assuring timely payment; record-keeping
Advance Directives: Health Care Proxy, Living Will, Power of Attorney
Working with Elder Law attorneys as needed

Eligibilty:

New York City; Westchester, Nassau, Rockland, Putnam, Duchess counties; and occasionally Connecticut

Fees:

Flat fee negotiated for psychosocial assessment negotiated, no time limit. Beyond assessment, fees charged hourly.
