Services:

Meals-on-Wheels: Delivers 7 nutritious meals per week to homebound elderly. Choices are hot meals, frozen meals and kosher meals. Included in the service are light shopping, chore and escort services.
Senior Center: Offers educational, health, recreational, social programs and social services, as well as breakfast and lunch to area seniors. Open Monday through Saturday.
NORC Supportive Services Program: Helps elderly tenants of the Isaacs/Holmes Houses remain in their own homes through coordination of needed services, such as case management, case assistance and other services to help residents lead independent and vital lives.
Elders at Home: A unique telephone conference call program that provides homebound seniors with socialization, education and recreation.
Case Management: Stanley Isaacs Neighborhood Center is part of the East Side Case Management Consortium which provides comprehensive case management services to homebound elders living on the Upper East Side and East Harlem communities. Referrals should be made to Intake, at Lenox Hill Neighborhood House at 212.744.5022 x1203.

Eligibilty:

All Programs: 60 years or older

Meals-on-Wheels:

Live on East Side from 59th Street to 142nd Street
Homebound or unable to attend congregate meal site
Unable to shop and prepare own meals
Must contact East Side Case Management Consortium for Intake at 212-744-5022, x1203.

Senior Center:

Resident of New York City

NORC:

Must live in the Isaacs/Holmes NYCHA buildings

Elders at Home:

Homebound

Case Management:

Live on East side from 59th Street to 142nd Street
Homebound

Fees:

No fees, but contributions are welcomed for meals
