Services:

Help with obtaining the wide range of entitlements needed by homebound, older people to live independently in the community; ongoing case management for people with no family or friends to provide this service; direct services not available from other providers include shopping, escort and chore services, daily money management, volunteer visitors, small group outings and luncheons and nursing services through affiliation with Hunter College.

Eligibilty:

Homebound; 60 years and over; East 86th to 106th Streets, Fifth Avenue to the East River.
